On your first visit for a consulation, we will go over any skin concerns, discuss treatment options and answer your questions.
What to Bring
- Completed patient information forms
- Name, address and phone number of your pharmacy
- Driver’s license
- CareCredit information (if applicable)
Arrive 10 minutes early
Please review directions to our building and plan to arrive 10 minutes before your appointment. Please arrive 30 minutes ahead of your appointment if you need to fill out your paperwork at the office.
